Billing / Payment Policies
As a courtesy to our patients, we will submit an itemized billing statement to your insurance carrier for the purpose of payment to Dr. David G. Madison, D.C., Chiroprcatic Corporation for health care services rendered.If you pay for your health care services directly, we will provide you with an itemized statement. The statement contains your medical information, including diagnosis, date of the injury or onset of the condition, and the procedure codes which describe the services received.
At times, additional information may be requested by the Insurance Company and will be provided to them. The doctor may have to write a detailed report regarding your condition, the treatment provided, an assessment of your future need for care, and any permanent impairment or disability that you may have. There may be a charge assessed for this type of report.
